Our mission is to help kids enjoy basketball in a safe and structured environment. Basketball teaches values kids will use the rest of their lives. It is our responsibility to emphasize these values to all of our kids and it is our goal to help every player experience success on the court and in the classroom.

I, the undersigned, hereby acknowledge that I am voluntarily registering my child in basketball/football/cheer organized by Horseshoe Bend Optimist Club. I understand that there are inherent risks involved
in playing basketball, football and cheer, such as injuries, accidents, or illnesses. I agree to assume all responsibility for any damages, liabilities, losses, or expenses that may incur as a result of my child’s participation. I/my child also agree to abide by the rules and regulations of the league and respect the decisions of the officials and organizers. I hereby release, waive, discharge, and covenant not to sue Horseshoe Bend Optimist Club, its officers, employees, agents, volunteers, sponsors, or affiliates, from any and all claims,
demands, actions, or causes of action arising out of or related to any injury, damage, or loss that my
child may suffer, or that my child’s next of kin may suffer, as a result of my child’s participation in football/cheer.Do we have permission to use images of your child on social media and this website?
